This year, the Mary Coram Unit focused on improving the patient journey, to make it seamless from initiation of a referral through to discharge or transfer of the patient.
Some of the areas identified as issues were:
- variations in process and management for admissions and discharge planning; and lack of communication with patients regarding discharge and process
- lack of consistent documentation and visible current patient information
- inconsistent management of referrals
By making improvements, the quality of service provision is enhanced for patients with Parkinson’s Disease and dementia, as well as patients requiring rehabilitation after orthopaedic hip or knee surgery or a stroke.
